Please note the proper procedures required. Players must get ref's signature and league director's signature as proof that it happened in league play, as well, a copy of the game sheet as additional proof. These documents with the claim form must be sent to CBHA office within 45 days of the injury. League should not be sitting on these forms. We have had several serious problems arise because of a few leagues who do not submit them on a timely basis. If a player loses their right to make a claim, and we have to settle it from CBHA funds, the league will be fined for the amount of the loss.
